Smart Ways to Compare Medicare Part D Plans During Open Enrollment
If you take even one prescription, choosing the right Medicare Part D plan can save you hundreds of dollars a year. Open Enrollment (October 15–December 7) is your main chance to switch, so it’s worth taking a careful, methodical approach instead of just auto-renewing.
Start With a Clean List of Your Drugs
Before you compare anything, make a current list of:
- Every prescription you take, including dosage and how often you refill
- Whether you use brand-name, generic, or are open to switching
- Your preferred pharmacies (including any mail-order options)
You’ll use this list in the Medicare Plan Finder tool or equivalent comparison tools offered by state health insurance programs.
Look Beyond the Premium
Many people choose a plan based on the monthly premium alone, then are surprised by high pharmacy bills. When comparing, look at:
- Total yearly cost estimate: premium + deductible + copays/coinsurance
- Deductible: how much you pay out of pocket before the plan helps
- Cost-sharing: your copay or coinsurance for each specific drug
A plan with a higher premium but lower copays for your medications can easily be cheaper overall.
Check the Formulary and Tiers Carefully
Each Part D plan has a formulary (its list of covered drugs) organized into tiers. As you compare:
- Confirm every drug you take is on the formulary
- Note the tier for each drug (lower tiers usually cost less)
- Watch for quantity limits, prior authorization, or step therapy rules
If a drug is not covered or is in a high tier, ask your prescriber whether a therapeutic alternative on a lower tier might work for you.
Compare Pharmacies and Networks
Plans contract with preferred pharmacies where you pay the lowest copays. When reviewing options:
- Check whether your regular pharmacy is in the preferred network
- Compare costs at different pharmacies shown in the tool
- Consider whether mail-order options would reduce your costs
If a plan looks good financially but doesn’t include your favorite pharmacy at preferred rates, weigh the savings against the inconvenience.
Consider How You Use Care Over a Full Year
Think about your likely needs in the coming year:
- Do you expect any new or expensive medications?
- Are you close to hitting the coverage gap (donut hole) in past years?
- Do you qualify for Extra Help or other assistance that affects costs?
Plans can also change their formularies each year, so even if your health is stable, your costs under the same plan may not be.
